OBJECTIVE
Review fall, falling-object, unsafe-access, incomplete-platform, overloading and unauthorised-alteration hazards, and verify the scaffold tag, inspection date and physical condition before use.
TOPIC EXPLANATION
A scaffold is a complete system consisting of platforms, top rails, mid-rails, toe boards, ladders, trapdoors, braces and ties. A single missing component can increase the risk of a fall or structural failure.
Scaffolds must only be erected, altered and dismantled by authorised scaffold personnel. Users must not remove or reposition platforms, rails, ladders or structural components for convenience.
Even when the tag is acceptable, the user must complete a visual inspection before every use. Untagged or red-tagged scaffolds must not be used, and yellow-tag conditions must be understood before access.
REALISTIC SITE SCENARIO
A team used a green-tagged scaffold to reach a pipe connection. During the task, a mid-rail was removed to pass material through the platform.
The rail was not replaced before the shift changed. The next team began work without noticing the missing component, and a worker lost balance near the open edge.
The event could have been prevented by prohibiting unauthorised alterations, reporting the defect immediately, closing the scaffold and arranging reinspection by the authorised scaffold team.
REMEMBER
A valid tag is not enough. Do not use a scaffold with missing, loose, damaged or altered components; report the condition immediately.

Team, before accessing the scaffold today, we will check the tag, inspection date, access, platforms, guardrails and trapdoors. We will not use the scaffold if the tag is missing, red or if any component is incomplete. No one may remove rails, decks or structural parts. Any defect must be reported and the scaffold kept out of use until checked by the authorised scaffold team.
