OBJECTIVE
Ensure that full-body harnesses, lanyards, energy absorbers and connectors are correctly selected, inspected and used, and prevent unsafe anchorage, disconnection and suspension-related risks.
TOPIC EXPLANATION
A safety harness is not a stand-alone item of PPE. The harness, connector, energy absorber, lifeline and anchor point form one fall-arrest system. If any component is unsuitable, the system may fail to protect the worker.
The harness must be adjusted correctly to the worker's body. Loose leg or shoulder straps can allow the worker to slip from the harness or suffer serious injury during a fall. Excessively tight straps may restrict movement and circulation.
The connection should be made as high as practicable and continuous attachment must be maintained. Fall clearance must include lanyard length, energy-absorber deployment, worker height and a safety margin above lower-level obstructions.
REALISTIC SITE SCENARIO
A worker wore a full-body harness for a short connection task on a pipe platform, but attached both hooks of the twin lanyard to unsuitable parking loops on the harness.
While moving between platforms, the worker disconnected completely. When the worker slipped, neither hook was attached to an anchor and the system could not arrest the fall.
The incident could have been prevented through pre-use inspection, correct connection training, continuous attachment and supervisor verification before work began.
REMEMBER
Wearing a harness does not provide protection by itself. Protection requires correct anchorage, suitable connecting equipment, continuous attachment and a practical rescue plan.

CONTROL MEASURES
Use only an approved full-body harness.
Inspect webbing, stitching, buckles, D-rings and labels before every use.
Verify periodic inspection status and records.
Adjust the harness correctly and evenly to the body.
Connect only to approved and adequately rated anchor points.
Select an anchor as high as reasonably practicable.
Maintain continuous attachment with a twin-leg lanyard where required.
Check available fall clearance and swing-fall exposure.
Protect equipment from sharp-edge contact.
Confirm the rescue plan, rescue team and equipment before starting.
SUPERVISOR TALKING SCRIPT
Team, today we will not wear a harness simply to meet a rule. We will inspect the webbing, buckles, D-rings, lanyard and hooks together. Connection will be made only to an approved anchor and continuous attachment will be maintained while moving. We will not start if fall clearance is inadequate, an edge is unprotected or the anchor is unsuitable. Everyone must understand the rescue arrangement.
