OBJECTIVE
Prevent high-risk work from starting until scope, hazards, isolations, simultaneous operations and required controls have been verified.
WHY IT MATTERS
A permit to work is not simply a signature document; it is a critical control system linking the task to actual field conditions.
The scope, equipment, location and controls stated on the permit must match the work being performed.
If conditions or work scope change, the existing permit may no longer be valid and the task must be reassessed.
REALISTIC SITE SCENARIO
A crew obtained a valid permit and later changed the work scope in the field.
The new task introduced an energy and equipment hazard that had not been assessed on the permit.
Stopping work and reassessing the permit scope would have prevented uncontrolled work.
REMEMBER
If the permit no longer matches the job, do not make the job fit the permit; stop and reassess.
