OBJECTIVE
Prevent process leaks caused by incorrect gaskets, misalignment, unsuitable bolts and incorrect tightening.
WHY IT MATTERS
Flange integrity depends on gasket selection, flange-face condition, alignment and the correct tightening sequence, not simply on bolts feeling tight.
Using bolts to pull misaligned flanges together can create permanent mechanical stress and increase leakage risk.
Incorrect bolt grade or lubrication can produce a clamping force very different from the intended torque value.
Scratches, corrosion or old gasket residue on flange faces can prevent the new gasket from sealing correctly.
During first pressurisation, personnel must remain clear of the potential leak path while the joint is monitored.
REALISTIC SITE SCENARIO
A flange was reassembled quickly after maintenance to meet a shift-change deadline.
Bolts were tightened sequentially rather than using the approved cross-pattern, creating uneven preload.
When the line was pressurised, leakage developed on one side of the joint.
Correct gasket verification, calibrated torque tools and an approved tightening sequence would have prevented the leak.
