OBJECTIVE
Prevent exposure to residual pressure, chemicals, gases and hot fluids while opening piping, flanges, valves, filters or process equipment.
WHY IT MATTERS
Trapped pressure or product residue can remain even after isolation.
Incorrect valve alignment or leaking isolation can expose workers directly to hazardous material.
The first break in containment is often the highest-risk step.
REALISTIC SITE SCENARIO
A crew began removing a blind flange from a line reported as isolated.
When the first bolt was loosened, trapped hot liquid sprayed through the flange gap.
Draining, venting, zero-energy verification and controlled first-break technique would have prevented the exposure.
REMEMBER
An isolation certificate does not replace physical verification of zero energy.
KEY HAZARDS
Residual pressure trapped inside piping.
Release of hot liquid or steam.
Exposure to corrosive or toxic chemicals.
Release of flammable gas or hydrocarbons.
Passing isolation valves.
Work on the wrong line or equipment.
Uncontrolled flange-bolt loosening.
Inadequate PPE for potential splash exposure.
CONTROLS
Physically verify the correct line and equipment.
Confirm isolation plan, LOTO and required positive isolation.
Drain and vent the system through suitable points.
Do not rely solely on a pressure gauge reading zero.
Perform gas testing where chemical or hydrocarbon service requires it.
Make the first break while keeping the body away from the potential release path.
Loosen the far side first where the approved procedure requires controlled flange opening.
Use chemical-compatible gloves, goggles, face shield and protective clothing.
Keep spill response equipment and emergency washing facilities ready.
Stop immediately if unexpected pressure or product is detected.
SUPERVISOR MESSAGE
Team, isolation alone is not proof of zero energy. We will verify the line, drain and vent it, and control the first break while keeping ourselves out of the release path.
