OBJECTIVE
Review injury, equipment-damage and production-loss hazards caused by tools, materials and equipment falling from height, scaffolds, platforms and lifting areas, and verify tethering, exclusion-zone and lower-area controls before work begins.
TOPIC EXPLANATION
Even a small nut, wrench or hand tool can cause serious injury when dropped from height. The severity depends on the object's weight, drop height, shape and impact point.
Tools must be secured with suitable tool lanyards, small parts kept in closed containers and loose materials kept away from platform edges. Toe boards alone do not eliminate every dropped-object hazard.
The area below must be barricaded and kept clear of personnel. Materials must be raised, lowered and passed using controlled methods and must never be thrown from one level to another.
REALISTIC SITE SCENARIO
A worker left a wrench close to the edge of a scaffold platform while completing a connection task. The tool was not secured.
While repositioning, the worker contacted the wrench with a foot and it fell to the level below. A nearby worker was exposed because the lower area had not been fully barricaded.
The incident could have been prevented through tool tethering, closed tool bags, effective exclusion control and a dropped-object inspection before starting.
REMEMBER
Every tool and material used at height can fall. Secure it, contain it, barricade the area and keep people out of the line of fire.
