Atmospheric Testing
- ✓Test oxygen first, then flammable and toxic gases.
- ✓Sample the top, middle and bottom of the space.
- ✓Record results on the permit and verify acceptable limits.
03MANDATORY
Energy Isolation
- ✓Identify every energy and material-flow source.
- ✓Apply LOTO, blanking or positive physical isolation.
- ✓Verify zero-energy status before entry.
04MANDATORY
Entry Attendant
- ✓Maintain a trained attendant throughout the entry.
- ✓Track the number and condition of all entrants.
- ✓The attendant must remain at the post without conflicting duties.
05WARNING
Ventilation
- ✓Use suitable ventilation supplying clean air.
- ✓Keep air intakes away from exhausts and contaminant sources.
- ✓Reassess entry immediately if ventilation stops.

Communication
- ✓Maintain continuous communication with the attendant.
- ✓Test radio, voice or visual methods before entry.
- ✓Evacuate immediately if communication is lost.

Rescue Readiness
- ✓Prepare a workable rescue plan before entry.
- ✓Ensure the rescue team and retrieval equipment are ready.
- ✓Never perform an unplanned or unprotected entry rescue.

Continuous Monitoring
- ✓Continuously monitor when atmospheric conditions may change.
- ✓Verify detector alarm settings before work starts.
- ✓Evacuate immediately after any alarm, symptom or concern.
